The Games have two objectives: to promote organ donation and encourage more people across the world to register and support donation; and to demonstrate to the society at large, that a thriving life is possible after a transplant.
Participation to the games is open to organ recipients, aged between 7 and 70, who have received organ transplants, such as heart, lung, kidney, liver, pancreas, or any other qualifying organ.
Living organ donors are also be eligible to participate.
Competitions are being held on 200m race, Swimming, Table Tennis, Basketball Shootout, Carroms, Chess, Bowling, Dart and Badminton, 5KM Racewalk.
Participants are required to have medical clearance from their healthcare providers, confirming that they are in good health and physically fit to participate in the chosen events.
Categories for competition are classified as Below 21 years, 21-50 years, above 50 years for Male and Female.

Family members of persons whose organs were donated, after brain death via Mrithasanjeevani project of Government of Kerala, are eligible to participate under this category.
Registration can be done only online. Registration forms for donor and recipient categories are available in the "Registaration" tab of the website.
Registration is mandatory for participation, but is free of cost and can be done online.
Two supporters of each donor/recipient can attend the games to applaud/encourage participants. Separate registration form is available for them “Supporter Registration” for such family members are mandatory.
Medals and certificates will be presented to all winners at the venue.
Volunteers are the pillars of strength for organizing any event. Any person interested can volunteer by registering under the “Volunteer" tab.
